Form to capture dates of Connect Church Staff Vacation Requests. Please complete to it's entirety and submit to be approved and placed on the Connect Internal Calendar.

For consideration: Ensure PCO has been updated with your vacation dates (blackouts) and you have an out of office set on your email with contact information of who is covering for you. Additionally, team members should be made aware of your absence and plans made to ensure coverage is adequate.
